H1N1 FLU CLINIC FOR STUDENTS TUESDAY, OCT 27, 11-3, October 21, 2009
Be sure to read the information provided by the local Health Department that was sent home this week.  Indicate on the form whether or not you wish your child to receive the swine flu vaccination that will be given at a clinic on Tuesday, October 27th in the Adrian Campus gym (lower level).  Montessori students receiving the vaccine will need a parent to bring them in person; students in grades 1-8 who are receiving the vaccine will be brought to the clinic by staff.  Remember that this clinic is for students and applicable staff only and is not open to the general public.  You can find up-to-the-minute information about the swine flu (symptoms, treatment, vaccine questions, etc.) at the website of the Centers for Disease Control website, cdc.gov.

UPDATE:
We just received word that the mist/nasal spray version of the H1N1 vaccination will be available (along with the shot form) at our clinic on October 27th.  The nasal spray form is different and parents should read about this difference at the Health Department Website here.

Parents MUST let us know if they want the nasal spray/mist form of the vaccination (instead of the shot) by a note attached to the official permission form. The note must clearly state parent's name, child's name, and a sentence saying that you want the child to receive the mist form of the vaccine.  We must receive the note along with the permission form no later than the end of the day on Friday, October 23rd.

A reminder that if you DO NOT want your student vaccinated at all you should fill in the "refusal" portion of the form we sent home and clearly indicate your name and your child's name.
